Home Visit/Parent Conference Form Guidance
POLICY:
NOCAC Head Start teachers will complete the NOCAC Child Development Home Visit/Parent Conference form as prescribed in the procedures below.
PROCEDURES:
- NOCAC Teachers need to fill out the entire top section of the Home Visit/Parent Teacher Conference form with accurate information and exact times.
- NOCAC Education Team will create a sample form before each Home Visit and Parent Teacher Conference. This will be sent out to the staff at least one week prior to the start date.
- Use complete sentences when filling out this form.
- When the visit is complete, both you and the parent will sign and date the form.
- Staff may not add information after the parent has signed
- Staff will document exact time of arrival and departure
- Staff may complete the following sections before the meeting with the parent/guardian
- What they are currently working on in the classroom
- Upcoming events
- Physical/Dental information
- Staff need to complete rest of the information on the form in the home
- The pink copy to the parent, yellow copy in the file at the site, white copy to the office.
- NOCAC Teachers will document information about the child on the NOCAC Child Development Home Visit/Parent Conference form as follows:
Child’s Progress and Education
- The following Progress Report was discussed:
- Initial Home Visit: Explain what is included in the progress report and when they will receive one.
- 2nd Home Visit and Conferences: Write which progress report was discussed (Fall, Winter, Spring)
- Child’s Transition:
- Initial Home Visit: Explain the classroom routine, times, and bus route. Ask the parent how they feel their child will transition into preschool.
- 1st Conference: Discuss the child’s strengths and transition into the routine/class.
- 2nd Home Visit: Discuss the child’s transition into the routine and to kindergarten (if applicable).
- 2nd Conference: Discuss the child’s transition into services the next school year.
- Document the child’s interests, what they have been doing in the class room
- Initial Home Visit: Note what the class will be working on, and discuss plans for cooking, field trips, guest speakers, or any other special events/activities.
- Conferences and 2nd Home Visit: Update the parents on what you are currently working on with the class. Discuss what Kindergarten readiness skills you are working on with their child. Discuss plans for cooking, field trips, guest speakers, or any other special events/activities.
- IEP/CSP Goal Progress:
- If no IEP, write NA.
- Initial Home Visit: Discuss IEP goals and how you will work on them in the classroom.
- 2nd Home Visit and Conferences: Document goal progress using the IEP objectives.
- New Goals from Parent:
- All Home Visits and Conferences: Encourage and assist the parents in creating goals for their child. This goal should then appear in your lesson plan through individualization or small group (required).
- Previous Parent Goal Progress:
- Initial Home Visit: NA
- 2nd Home Visit and Conferences: Discuss progress to the previous goals.
- Other:
- Document any other information given, discussions, or paperwork filled out.
- Initial Home Visit: Review Creative Curriculum and DRDP with the families explaining the objectives we will be working on.
Health/Screenings
- The following Assessments were discussed:
- Initial Home Visit: Explain what all of these are, when they will be done, and then checkmark the boxes.
- The parent will complete the Social/Emotional Screening (Ages and Stages) at the initial home visit if not completed before this time. The ASQ 3 will be complete with the family.
- 2nd Home Visit and Conferences: Checkmark the assessments/screenings that were gone over with the family.
- Document any notes in the “Other” section.
- Initial Home Visit: Explain what all of these are, when they will be done, and then checkmark the boxes.
- Health Services:
- Initial Home Visit: Document any special needs for school and give physical/dental updates. Note any health updates.
- 2nd Home Visit and Conferences: Document progress on special needs in school and give physical/dental reminders when necessary. Note any health updates.
- Other:
- Document any other information given, discussions, or paperwork filled out.
Family Engagement
- The following were completed or reviewed:
- Initial Home Visit: Discuss inkinds, bag tags, and Class Dojo and complete the Family Interest Survey.
- 1st Conference: Discuss inkinds, bag tags, and Class Dojo. Discuss and complete the Family Needs Assessment, and Family Partnership Agreement.
- 2nd Home Visit and Conferences: Discuss inkinds, bag tags, and Family Central. Review the Family Partnership Agreement and complete Follow Up as needed.
- The following Family Engagement and Volunteer Opportunities were discussed:
- Initial Home Visit: Discuss any events and encourage participation. List topic, date, time, and locations if known.
- Document and discuss specific ways parents can volunteer in the first few months.
- Encourage them to attend and ask if they are interested in setting a date.
- Go over the volunteering section in the Parent Handbook.
- 2nd Home Visit and Conferences: Discuss any events and encourage participation. List topic, date, time, and locations if known.
- Document and discuss specific ways parents can volunteer.

- Thank parents for any volunteering they have done.
- Initial Home Visit: Discuss any events and encourage participation. List topic, date, time, and locations if known.
- Parent/Guardian Suggestions:
- All Home Visits and Conference: Ask the parent for any suggestions for the classroom (activities, field trips, guest speakers, cooking experiences, etc.) or the program.
- Emergency/health/personal information:
- All Home Visits and Conferences: Document any forms completed or changes made. Make changes on the yellow/white Child Enrollment and Health Information Form (form H3 – Child Enrollment and Health Information), Authorized Pick Up List (form T8 – Authorized Pick Up List) and emergency contact cards. If no changes were made, check “up to date”.
- It is best practice to take the Child Enrollment and Health Information form to Home Visits and PT Conference for the parent to review to be sure all information is still accurate.
- All Home Visits and Conferences: Document any forms completed or changes made. Make changes on the yellow/white Child Enrollment and Health Information Form (form H3 – Child Enrollment and Health Information), Authorized Pick Up List (form T8 – Authorized Pick Up List) and emergency contact cards. If no changes were made, check “up to date”.
- Other:
- Initial Home Visit:
- Document that the Family Handbook and Calendar was reviewed and discussed.
- Discuss what they will find in the classroom newsletter.
- Document completion of Transportation Orientation Form (form T11 – Transportation Orientation Form).
- Document any other information given, discussions, or paperwork filled out.
- 2nd Home Visit and Conferences:
