IEP – Kindergarten Transition – Additional Services

POLICY:

NOCAC Head Start will plan and implement additional transition services for children and families with IEPs.

PROCEDURES:

Teachers will:

1. Provide the Introduce Me! Handout to families of children with IEPs to be distributed and completed at the final parent/teacher conference in order for families to share with the kindergarten teacher.
2. Attend all IEP transition to kindergarten meetings scheduled by the LEA to support the child and family as they transition to a new setting.
