POLICY:

NOCAC Head Start collaborates and communicates with families in accordance with the requirements.

PROCEDURES:

  1. For activities described in 1302.33, NOCAC Head Start will collaborate with families as partners in the health and well-being of their children in a linguistically and culturally appropriate manner and communicates with families about their child’s health needs and developmental concerns in a timely and effective manner.
  2. At a minimum NOCAC Head Start will obtain advance authorization from the parent or other person with legal authority for all health and developmental procedures administered through the program or by contract or agreement and maintains written documentation if they refuse to give authorization.
    1. The NOCAC Head Start Enrollment Consent Form will be reviewed and completed at the first home visit with the family.
      1. The teacher or home visitor will retain the yellow copy in the child’s center file.
      2. The teacher or home visitor will send the white the copy to the Central Office for processing by the Administrative Assistant.
        1. The Administrative Assistant will document the information in ChildPlus.
