Permanent Supportive Housing Specialist
Location: Northwestern Ohio
Department: Homeless Programs
Reports To: Homeless Program Manager
Schedule: 34 hours/week, Monday–Thursday (Full Year)
Pay Range: $19.40–$20.80/hour (based on educational qualifications)

We’re looking for a Permanent Supportive Housing Specialist to join our compassionate, mission-driven team. In this role, you’ll help people facing chronic homelessness find stability, build independence, and connect to vital community resources.
What You’ll Do

Partner with individuals and families experiencing homelessness to find and maintain safe, affordable housing.

Build and sustain relationships with local landlords, property managers, and community organizations.

Connect clients to essential supports such as mental health care, employment services, financial counseling, and healthcare.

Use a Housing First approach that emphasizes dignity, client choice, and community integration.

Maintain accurate case records and outcome data in the Homeless Management Information System (HMIS).

Provide advocacy, guidance, and crisis intervention when challenges arise to help prevent eviction and ensure long-term housing stability.
What You Bring

Associate’s degree in Social Work or a related field (Bachelor’s preferred).

Knowledge of case management, social services, and community resources.

Strong communication, empathy, and organizational skills.

Valid driver’s license and insurability under NOCAC’s policy.
